Service and Maintenance Agreement Information
FHC has a limited loaner inventory available at no charge for customers whose products were purchased within the last year, or are covered by an extended service warranty. Since the loaner inventory is limited and available on a first come-first served basis, we discourage use of these for anything other than to support a surgery during an unanticipated repair.
Customers whose equipment is not covered by an extended warranty will be charged a flat fee and assigned a return date. Equipment not returned when scheduled will be subject to the additional loaner fees for each subsequent week until it is received.
For annual preventive maintenance, please call or email to reserve a service appointment during a break in your surgical schedule when you will not require a loaner.

Please call or email to initiate this process. If we determine that your product needs to be returned to the factory for service or for annual Preventive Maintenance you will be asked to:
- Issue a Purchase Order (Products covered by an existing warranty will be repaired at no cost)
- Complete a Statement of Decontamination (Click here to download a PDF)
- Provide the name, email, and phone number of a contact for all repair related questions
- Provide a detailed description of any specific issues you would like us to address
- Carefully package your equipment for shipment
- Label package with the RMA# (provided to you by FHC), and attach the Statement of Decontamination to the OUTSIDE of your package
